The types of back pain we see most
“Back pain” is a broad category. The specific kind you have determines what’s wrong and what we need to do about it. The most common patterns:
- Low back pain that started suddenly, usually from lifting wrong, twisting awkwardly, or sleeping in a strange position. This is often a joint or muscle issue that responds quickly to care.
- Chronic low back pain that’s been around for months or years. Usually involves multiple factors (joint dysfunction, muscle imbalance, disc degeneration) and takes a more comprehensive plan.
- Mid back pain between the shoulder blades, almost always related to posture, desk work, and rib joint dysfunction.
- Lower back pain that shoots down the leg, that’s sciatica, not just back pain. We have a separate page on sciatica with the specific approach.
- Morning stiffness that loosens up as the day goes on, often related to disc dehydration or inflammation patterns.
- Pain after activity, usually a muscle, joint, or stability issue that shows up when you load the spine.
How we treat back pain
The treatment depends entirely on what’s causing your specific pain. The first visit is built around figuring that out so we know what to do.
Spinal adjustments for joint dysfunction, restricted segments, and disc-related compression. This is the foundation of chiropractic care for back pain. Done correctly, it’s gentle and feels surprisingly good.
Dry needling when the pain has a strong muscle trigger point component. Most chronic back pain has muscle involvement that limits how well joint-only treatment works. Releasing those trigger points often unlocks the rest of the progress.
Acupuncture for chronic inflammatory cases, or when stress and tension are amplifying the muscle component.
Movement coaching. Most back pain is partly a movement habit problem (how you sit, lift, sleep, stand). If we don’t address that, the pain comes back. We’ll teach you the changes that matter most for your specific case.
What to expect at your first visit
- History. When did it start? What makes it worse? What makes it better? Where exactly does it hurt? Has anything like this happened before?
- Exam. Range of motion testing, orthopedic tests, muscle palpation, neurological screening if there’s any radiating symptoms. Usually 10 to 15 minutes.
- Treatment plan. Specific explanation of what’s causing your pain, what we’re going to do, how many visits, and what improvement to expect.
- First treatment. Most people get adjusted and/or needled at the first visit. Many feel noticeably better walking out, though durable change builds over the first few visits.
How long until I feel better?
Acute back pain (recent flare-up, less than 4 weeks old) usually responds in 4 to 6 visits over 2 to 3 weeks.
Chronic back pain (months or years) takes longer, usually 8 to 16 visits over 6 to 12 weeks, sometimes with periodic maintenance after that.
If your case involves serious disc degeneration, arthritis, or other structural issues, the plan will be longer and the goal becomes managing the condition well, not “curing” it.
When back pain needs more than a chiropractor
Most back pain is mechanical and responds to conservative care. Some situations need different attention right away:
- Sudden, severe back pain after a fall or accident
- Loss of bowel or bladder control
- Numbness in the saddle area (groin, inner thighs)
- Unexplained weight loss with back pain
- Fever with back pain
- Pain that’s significantly worse at night and doesn’t change with position

Frequently asked questions
Should I see a chiropractor or my regular doctor for back pain?
For mechanical back pain (the kind that started with lifting, sleeping wrong, sitting too long, etc.), evidence supports trying chiropractic care first. It’s effective, low-risk, and avoids the side effects of pain medication. Your regular doctor is the right call if you have any of the red flag symptoms listed above, or if conservative care isn’t moving the needle.
Will my insurance cover it?
Most major insurance plans cover chiropractic care for back pain, often with a copay. We verify your benefits before the first visit so you know exactly what’s covered. HSA and FSA cards also work.
Do I need an MRI before I can be treated?
Almost never. The orthopedic and neurological exam at your first visit tells us what we need to know to start treatment safely. If something doesn’t add up or you’re not responding to care, we’ll refer for imaging.
Is it safe to get adjusted if I have a disc issue?
For most disc issues, yes. We use specific techniques designed for disc cases, not the same approach as for a stiff joint. The exam tells us what your spine can handle. We don’t do anything that’s not safe.
How is dry needling different from a regular adjustment?
Adjustments work on joints. Dry needling works on muscle trigger points. Both can be part of back pain treatment. Adjustments handle the joint side, needling handles the muscle side. Most chronic back pain involves both, which is why a combined approach usually works better than either alone.
Will my back pain come back?
If we only treat the immediate flare-up and don’t address what caused it (how you sit, how you sleep, what your activity looks like), recurrence is likely. The reason we include movement coaching is to make the result stick. Most patients who follow through on the home plan don’t deal with the same pain again.
What if I’ve had back pain for years?
Long-standing back pain is still treatable, it just takes longer and the plan is more comprehensive. Many of our chronic back pain patients had given up before coming in. We’ll set realistic expectations at the first visit and you’ll know after the first few weeks whether this is working for you.
